FREE Flu Shots by Appointment

Appointments can be scheduled using the Healthy Coog Patient Portal.

Due to limited space and social distancing, we ask that you complete all forms  prior to your arrival.

1. Arrive to your visit about 5 minutes prior to your scheduled time.
2. Print, complete and sign the Flu Vaccine Consent Form on Friday, September 24th and bring to your appointment. Click here for Form. 
3. Print, complete and sign the COVID-19 Symptom Assessment Form on Friday, September 24th and bring to your appointment. Click here for Form.
4. Wear clothing that will allow you expose your upper arm to receive your flu shot.
5. Those arriving late for their appointment or without completed forms may be asked to wait until a later time.
