Trained Community Health Workers (CHWs) provide a peer-based relationship with household members in the East End and Third Ward neighborhoods. They connect residents with necessary health and social services such as health coverage, medical and mental health services, food, and other social needs. The CHWs focus on building relationships and trust to improve health and health care access for members.
