Household-Centered Care Program

hcc-thumb.pngThe Household-Centered Care (HCC) Program at the Tilman J. Fertitta Family College of Medicine works to improve health outcomes of families in neighboring communities with some of the poorest health indicators in Harris County - the East End and Third Ward. This program has two key components:

  1. Healthy Connections | Conexiónes Saludables 
    The program provides a FREE resource navigation home visitation program to community members in neighboring communities. Community Health Workers and students visit participants in the comfort of their home to provide health education, resource navigation to participants and their family with local health, social and community services.

  2. Interprofessional Student Teams
    The program brings together interprofessional teams composed of community health workers, students from UH Andy & Barbara Gessner College of Nursing, UHD’s Bachelor of Social Work and Tilman J. Fertitta Family College of Medicine. 

The combined efforts of Community Health Workers from Healthy Connections and interprofessional teams of nursing, medical and social work students and faculty, allow us to promote coordinated care to address community health and social service needs.
