Medical Records

All information is considered confidential and will not be released without the patient's written consent or a signed court order. The Student Health Center retains medical records for 10 years past the last date on which the service was given (22 TAC §§ 165).

Your medical records may include history, diagnoses, and/or treatment of drug or alcohol abuse, mental illness, or communicable disease, including Human Immunodeficiency Virus (HIV) and Acquired Immune Deficiency Syndrome (AIDS).

There is no cost for a requests for your records to be sent to another healthcare facility or for an initial copy for your personal use. Records requested for insurance, legal purposes, other non-healthcare purposes or multiple requests for personal use may require a retrieval fee to be paid prior to processing.

Medical records are destroyed 10 years after the last date of service.

To request copies of your records

Print, complete, and physically sign an Authorization for Release of Medical Records Form. Electronic/Digital signatures cannot be accepted.

Submission Instructions:

IN-PERSON: Submit the completed form at the UH Student Health Center front desk. A valid picture ID must be presented at the time of submission.

MAIL: Submit the completed form by mail with a legible copy of your driver's license to the UH Student Health Center.

FAX: Submit the completed form by fax to the UH Student Health Center with a legible copy of your driver's license.

UPLOAD: Submit the completed form in PDF format through our Healthy Coog Patient Portal. Currently enrolled students only.

Due to privacy regulations, we are unable to accept submissions by email.

The UH Student Health Center will process an authorized request for medical records within approximately seven to fifteen business days after the receipt of a valid request.

  • Request to another healthcare facility will be faxed whenever possible.
  • Copies of large charts or records requests for personal use will be sent by mail or held for pick-up.
  • Records cannot be emailed.

Medical records are destroyed 10 years after the last date of service.

Important Notice Regarding Social Security Numbers

UH Student Health Center medical records generated prior to January 2007 or from outside providers could contain your social security number (SSN) and may be disclosed with your medical records.
