Adding Personalized Questions

How the "Additional Personalized Questions" Option Works:

  • Faculty can submit up to five additional questions for evaluation forms to their section(s) before the evaluation period begins for each session. 
  • Please go to AccessUH, click the red icon for faculty/course evaluation, and click the "Faculty—Submit Additional Questions" button.
  • The "Submit Additional Questions" button will only be visible for courses for which evaluation has not started.
  • After submission, these questions are sent for approval to the Approver (Chairs or Directors).
  • The Approver has the following actions available:
    • Approve: If approved, the question is added to the evaluation form.
    • Reject: If rejected, the faculty can submit a different question.
    • Modify: The Approver can modify the question and send it back to the instructor for approval.
  • If the faculty agrees with the modified question, it will be added to the evaluation form.
  • If the faculty disagrees, they can submit another question for approval.

The process ensures that questions meet the required standards before being added to the evaluation form.

Faculty members may also select question(s) that meet their needs from the PICES catalog of Likert-scale items which contains 607 questions. The University of Houston received permission from Purdue University to use the PICES catalog.
