Please fill out the application form and email it to
cccuh@mail.uh.edu by clicking the email
button in the PDF window and selecting "Send Copy". You may have to save the completed application to your computer and send it as an attachment.
You may also print a copy and drop it off at the CCC office (Bldg. 523, Room 103) or mail it to us.
Please note that your membership application is not valid until you have paid your
annual membership fee of $20.00.
