Faculty, staff, students, and community members are encouraged to submit an incident report when a student’s behavior becomes concerning and may be impacting their academic success. Concerns about absences, hospitalizations, death, mental health, etc. may be reported here. If you would like to first consult with a staff member before submitting a report, please call the Dean of Students Office at 832-842-6183. 

Please note that review of these forms will not take place outside of normal business hours. If there is an immediate risk to health or safety, please call University of Houston Police Department at 713-743-3333.
