Faculty/Staff Vaccination Incentive Program

The purpose of the program is to mitigate the spread of COVID-19 on the UH campus by increasing vaccination rates among faculty, staff and students. Vaccination remains the best intervention for reliable protection.

If you have not been vaccinated, familiarize yourself with the pertinent information, consult with your doctor and seriously consider becoming vaccinated.

Incentives

All faculty and staff who voluntarily participate in the Cougar Vaccination Program will receive $50 in ShastaBUCKS, which can be used via your Cougar Card at on-campus dining locations, the Campus Store, vending machines and select off-campus merchants.

You may be eligible for other incentives:

  • 10 randomly selected participants have their faculty/staff parking permits paid for the 2021-22 academic year.
  • If you do not have a parking permit and do not plan to purchase one and are randomly selected for the parking incentive, you will receive $445 in ShastaBUCKS.

Eligibility:

  • Every faculty and staff member who is employed at the University of Houston is eligible to participate. The program is completely voluntary.

  • The $50 in ShastaBUCKS incentive will be distributed beginning the week of August 23, 2021, after verification of documentation is confirmed, and will continue through early October 2021.

  • Once verified, each participant automatically becomes eligible to participate in the parking permit incentive if they are employed at UH as of September 30, 2021. This incentive will be awarded the week of October 4, 2021, and Parking and Transportation staff will contact recipients directly.
  • Participants will be eligible for the additional incentives that align with their primary function or role: employee OR student. Participants who are full time employees are only eligible to receive additional employee incentives.

Vaccination Incentive Program FAQ's

The purpose of the program is to mitigate the spread of COVID-19 on the UH campus by increasing vaccination rates among faculty, staff and students. Vaccination remains the best intervention for reliable protection. The program offers an incentive for unvaccinated individuals to get the vaccine and rewards those who have already been vaccinated. We respect the choice of those members of our community who cannot or are choosing not to be vaccinated.

No, participation is voluntary. Only those who choose to participate by submitting appropriate documentation will be eligible to receive an incentive.

All currently enrolled students and currently employed faculty and staff who choose to participate and submit appropriate documentation through the Cougar Vaccination Incentive Program icon on AccessUH by September 30, 2021 are eligible to receive incentives.

Yes, current students, faculty and staff who are fully vaccinated and submit appropriate documentation through the Cougar Vaccination Incentive Program icon on AccessUH by September 30, 2021, are eligible to receive incentives.

Individuals are fully vaccinated if they received either:

(a) two doses of Moderna or Pfizer;

(b) one dose of Johnson and Johnson, or;

(c) a full vaccination series of COVID-19 vaccines authorized for emergency use by the World Health Organization per U.S. Center for Disease Control (CDC) guidance will be eligible for incentives.

Contact your vaccination provider, such as the pharmacy, hospital or a local government office.

The Student Health Center does not maintain a copy of vaccination obtained through the mass vaccination clinics that were held on campus. Use the following contact information if you received your COVID-19 vaccination from one of the mass vaccination clinics and need a copy of your COVID-19 Vaccination Record.

  • Walgreens (Moderna or Pfizer):
    • Contact your local area Walgreens Pharmacy or call 1-800-WALGREENS (1-800-925-4733).
  • HEB (Johnson & Johnson):
    • Contact your local area HEB Pharmacy or call 1-800-811-8620.
  • Memorial Hermann (Pfizer):
    • Contact Memorial Hermann at 833-772-2864

To participate in the Cougar Vaccination Incentive Program using vaccination records, the documentation must show that the individual received the full vaccination series.  Individuals who received either:

(a) two doses of Moderna or Pfizer;

(b) one dose of Johnson and Johnson, or;

(c) a full vaccination series of COVID-19 vaccines authorized for emergency use by the World Health Organization per US. Center for Disease Control (CDC) guidance will be eligible for incentives.

To be eligible to participate you must have received the full dosage of a vaccine authorized for emergency use by the U.S. Food and Drug Administration (FDA) OR listed for emergency use by the World Health Organization (WHO). Per the Center for Disease Control (CDC), individuals who received a full vaccination series of COVID-19 vaccines authorized by the FDA or WHO are considered “fully vaccinated;” therefore, they would be eligible for the VIP program.

Each participant is eligible to receive the $50 in ShastaBUCKS incentive 1 (one) time only. The participants will be eligible for the additional incentives that align with their primary function or role: employee OR student. Participants who are full time employees are only eligible to receive employee incentives.

No. Once your documentation is validated, the record is destroyed and removed from the system.

If you are randomly selected to receive one of the additional incentives, you will receive an email from a university representative during the week of Oct. 4.

Outside the $50 in ShastaBUCKS that each participant will receive, participants are only eligible for one additional incentive.

No, the incentive is exclusively for the winner who is randomly selected and is not transferrable. While you can decline an incentive, once your name is drawn it is not added back to the pool.

Please follow these steps to verify and confirm the disbursement of $50 ShastaBUCKS:

  • Log into AccessUH,
  • Click the Cougar Card Online icon.
  • Click on Financials.
  • Click on Balances.
  • Verify that the funds are received. The funds information should be available under “ShastaBUCKS”.

We are still working to determine the tax implications, if any, and hope to provide the most accurate information possible.

Employee additional incentives are likely taxable personal benefits which will be reflected on an employee's Form W-2 as wages and/or may be reported on a Form 1042-S (international faculty, and staff).

A recipient information form will be required to be completed by the selected participant before the incentive is received.
