5 STEPS TO STUDY ABROAD!

STEP 1.

• Schedule an apppointment with Study Abroad Advisor or Director
• Submit to advisor your study abroad on-line application form
• Research availability of study abroad programs; choose your courses and provider or university

STEP 2.

• Log in to complete your Course Equivalency Form on-line and print it out.
• Meet your study abroad advisor Maria J. Saldana at 501 of E-cullen Building, Click for map.
• Follow her instructions for the UH study abroad process.
• The following non-refundable fees for services will be posted to your PeopleSoft account based on your program:
  For Affiliated Program: $150, for Special Program: $150, for Visiting International Studies Program (VISP): $150, for Exchange Outgoing Program: $125 and for Faculty Led Program: $50.
• Enroll with provider or university and submit the acceptance letter.

STEP 3.

The following documents should be completed at the OISP office- Resource Center.

• Get your Photographs taken (at OISP)
• Complete, sign and submit (at OISP):
  Health Disclosure Form
  Affidavit Form
  State Registration Form
• Read, sign and submit (at OISP):
  Release and Waiver of Liability Agreement
  Consent to Release Information (notarize at OISP)
• Submit to OISP a copy of you Passport page and Visa (if applicable)
• Get your ISIC Card (from OISP)

STEP 4.

• Meet with the Director Ms Parul Fernandes to sign off for registration
• Attend Pre-departure Orientation (Mandatory)
• Mandatory Reading

STEP 5. Re-entry (Grades will not be entered till re-entry in completed)

• Submit the essay and Re-entry Evaluation Form to your Study Abroad Advisor.
• Schedule an apppointment with Director, Ms. Parul Fernandes.

YOU MUST COMPLETE YOUR FILE BY THE FOLLOWING DEADLINE DATES:

For Spring Semester 2014: November 15, 2013

Summer Semester ( l, ll & lll ) 2013: April 19, 2013

Summer Semester ( IV ) 2013: May 31, 2013

Fall Semester 2013: June 28, 2012
