The forms given below are to help you with the registration of the Study Abroad program. Every student needs to fill in the course equivalency for before registration. The form should indicate the equivalency between the UH courses and the courses to be taken abroad. This form also needs to be signed by the student and the advisor/faculty member before submitting it to the Director of OISP. Detail instructions are given below.1)
